To complete your Executive Coach Accreditation Live Assessment, you’ll need to submit specific documents after booking your session and prior to attending the live session. These documents verify your training, coaching experience,
and professional development.

Documents You’ll Need to Provide
Before your live assessment session, you’ll need to submit the following:
- Coach Training Certificate – Evidence of completed training hours
- Coaching Experience Log – Record of your coaching delivery
- Client Reference(s) – Completed AC reference form from client(s)
- Coach Supervision Report – Includes supervision log, learning record & supervisor’s reference
- Continuous Professional Development (CPD) Certificate(s) – Evidence of ongoing professional development
Please refer to the overview for required hours and the number of supervision sessions, client references and CPD certificates needed at each level.
Key Information
- Please upload all documents in advance so we can review your application and confirm your place. Without them, we won’t be able to proceed with your assessment.
- Some documents require you to complete AC templates (which can be found below), while others (such as training certificates) are external documents you can gather from your records.
- During the session, you'll complete a development plan, please review the AC Development Plan in advance and have it ready on the day.
- Our team will contact you after booking your live assessment session with instructions on how to submit your documents.
Supporting Documents & Pre-Reading
Coach Training Certificate
- Certificates need to meet the required training hours for your accreditation level.
- You may submit multiple certificates if your training was completed across different providers.
Coach Experience Log
- Use the AC Coaching Experience Log Template to record your coaching delivery hours.
- The log includes guidance on how to structure and report your coaching experience.
Client Reference
- Ask your client/s to complete the AC Client Reference Form template and to return it back you.
- Ensure you have the correct number of references needed for the level you are accrediting at and that your clients complete all sections.
Coach Supervision Report
- Complete the AC Supervision Report Template, include learnings from your coaching supervision.
- Your coach supervisor will also need to complete part of this form, so allow time for this.
- Ensure your supervision sessions meet the requirements, are evenly spaced, and your most recent session is close to your assessment date.
CPD Certificate
- CPD can include events, conferences or workshops such as an AC Webinar. If you don’t have a certificate, an email confirmation of attendance will suffice.
- You’ll need to upload one CPD certificate for all levels expect Master level where you’ll need two certificates
